Passports

U.S. passports, either in book or card format, are issued only to U.S. citizens or non-citizen nationals.  Each person must obtain his or her own passport book or passport card.  The passport card is a U.S. passport issued in card format.  Like the traditional passport book, it reflects the bearer's origin, identity, and nationality and is subject to existing passport laws and regulations.  Unlike the passport book, the passport card is valid only for international travel by land or sea between the United States, Canada, Mexico, the Caribbean, and Bermuda.  It is not valid for travel by air to or from any foreigh destination.

FIRST TIME APPLICANTS 16 YEARS OF AGE AND OLDER

  • Completed passport form DS-11, do not sign until you come in to apply
  • Certified copy of birth certificate, filed within one year of birth
  • 2 passport photos
  • Driver's license
  • Fees
NOTE: Applicants who are 16 or 17 years of age will also need one of their parents to come in and sign the application.

APPLICANTS UNDER 16 YEARS OF AGE

  • Completed passport form DS-11
  • Certified copy of birth certificate
  • 2 passport photos
  • Both parents must come in with the child.  We will need both parents' driver's licenses.  If both parents cannot come in, the non-applying parent will have to fill out form DS-3053 (statement of consent), and have their signature notarized.  The applying parent can fill out section 5 of this form when the written consent of the non-applying parent cannot be obtained.

APPLICANTS WHO HAVE HAD A PREVIOUS PASSPORT

Use passport form DS-82 if you can comply with the following requirements:

  • You are able to submit your most recent U.S. passport book or card
  • You were at least 16 when your most recent passport book or card was issued
  • Your passport book or card was issued less than 15 years ago
  • You use the same name as on your most recent passport book or card or your name was changed by marriage or court order and you can submit proper documentation to reflect your name change

If you meet all of the above requirements, send in the documents listed below to the address listed on the form.

You will need to send in all of the following:

  • Completed passport form DS-82
  • Old U.S. passport
  • 2 new passport photos
  • Fee

If you do not meet all of the requirements for using form DS-82, then you will have to use form DS-11 and apply in person with all the documentation listed above for first time applicants.  However, you will use your old passport instead of your birth certificate for proof of citizenship.

EXPEDITED SERVICE - $60 Expedite Processing

Passport books and cards are good for 10 years for applicants 16 years of age and older.  They are good for 5 years for applicants under 16 years of age.

You should receive your passport 4 to 6 weeks from the date you apply.  You can get your passport 2 to 3 weeks from the date you apply if you pay an additional fee for expedited service.  If you are leaving within the next two weeks you can make an appointment with the passport agency in Seattle and pick up your passport there.

Your proof of citizenship, (birth certificate, naturalization papers, or old passport), will be returned to you in a separate envelope from your new passport.
